Outside Rome, the country is not primarily a tourist economy. The vineyard, the riding school, the cave guide and the cookery teacher work mostly with Italians, which is exactly what makes the day good and also why the language question is a real one.
This page lists the operators who have told us they work in English. It is an honest subset rather than a complete one: many others do too and simply have not said so anywhere we can check, and we would rather show you a short verified list than a long invented one.
For everything else, the practical answer is to ask. Every card on this site carries the operator's own phone number where we have it, and one message in advance settles it. Guided visits at the bigger sites usually have English at fixed times; small operators will often arrange it if they know you are coming.
The first grid is the one to use if you are deciding tonight for tomorrow, because it applies both filters at once: the operator has told us they work in English, and they take a booking online. That is the shortest path there is from a hotel room to a confirmed day out, and it is a short list on purpose.
For everything else, the practical technique is a message rather than a search. Small operators here often work in English perfectly well and have simply never written it on a website — the guide learned it, or the daughter handles the visitors. One message the day before, in English, gets you an answer and usually gets you a warmer welcome than arriving unannounced.
Two things are worth knowing about how language actually plays out on these days. Anything physical — kayaking, riding, a cave, a cookery class — needs far less language than you would think, because the instruction is demonstrated. Anything historical needs a great deal, because the whole value is what the person is telling you. Book the guide in English; worry less about the instructor.

Because we only list what the operator has actually told us. Plenty of others work in English and simply have not written it anywhere we can check, and we do not publish a language we have not seen stated.
A number of operators declare German, French or Spanish as well — usually the ones on the coast and in the wine country. If that is what you need, call the number on the card and ask; it is a quicker answer than any list.

Several operators declare French, German or Spanish as well, usually on the coast and in the wine country, and a number of the licensed town guides work in more than one. Ask on the card's number rather than working from a list: it is a faster and more reliable answer.
Licensed guides in the Tuscia and the Castelli take day bookings and several declare English, and a hired car with a driver is arranged separately from the activity. Booking the two together is what makes a long day out of Rome work without any timetable in it.
