There are two kinds of thermal day near Rome and they are not the same experience. One is a free open-air pool where sulphur water runs over rock and there is nothing there but the water — Viterbo's Bullicame is the famous one. The other is a spa: changing rooms, several pools at different temperatures, a restaurant, a booking.
Both are on this page. The water is genuinely hot in all of them, it smells of sulphur in most of them, and the best time of year is the one nobody expects — a cold clear day in winter, with steam coming off the surface.
Choose between the two kinds by what you want the day to be. A free open-air spring is an hour or two, costs nothing, and is a slightly wild thing: hot water running over rock into pools, no attendant, no lockers, no lifeguard, and whoever else has had the same idea. A spa is a day: several pools, a temperature you choose, a changing room, a restaurant and a booking. Neither is the superior version — they are different afternoons.
Geography splits the same way. Tivoli's sulphur lakes sit east of Rome and the springs at Cretone and Stigliano north of it, all within an hour. Viterbo is ninety minutes and is the concentration: free pools out in the open on one side of the town, and established baths on the other. Saturnia, in the Maremma, is the long day — the water falls down a staircase of white basins it built for itself.
The practical list is short and it matters. A towel per person, flip-flops for hot wet rock, a bag for the swimsuit that will smell of sulphur for days, and no silver jewellery unless you want it to go black. Drink water: hot mineral water is more dehydrating than it feels.
Then the part people get wrong about timing. These are winter places. A cold clear afternoon with steam standing over the surface is the version everybody remembers; a hot August day in water that never cools down is endurance. Midweek out of season is when you get a pool to yourself.
Tivoli's sulphur lakes on one side of the city and the Stigliano and Cretone springs on the other, with the Ferentino springs to the south. If what you want is a private warm room rather than open water, Ariccia has a cave spa at about the same distance — a different kind of day, not a thermal spring. Close enough that the drive is not the point of the day, and several sell a plain day ticket rather than a treatment.

More than for most days out. You come out of hot sulphur water warm, slow and slightly dazed, which is a poor state to start an hour of driving in. It is one of the few days where paying somebody else to drive is not a luxury.

At the spas usually yes, sometimes with an age limit and sometimes with restrictions on the hottest pools; the operator sets it and the card carries it. The free open-air springs have no supervision at all, which is the thing to weigh rather than the water itself.
A towel, flip-flops, a swimsuit you do not mind, a plastic bag for it afterwards, and drinking water. Leave silver jewellery at the hotel — sulphur tarnishes it. At the free pools, take everything you will need, because there is nothing there but the water.
